Follow-up for routine inspection conducted on 7/2/25 where one live cockroach was observed. At time of inspection, did not observe any cockroach activity or evidence of infestation. Hole in wall above the water heater has not been sealed - facility shall continue to seal and eliminate any gaps, openings, or crevices to remove points of harborage for vermin.

Routine inspection conducted in conjunction with CO0157239.

    1) Front door is kept open at time of inspection. [CA] Keep door closed to prevent the entrance of vermin. 2) Hole is observed on wall above the water heater. [CA] Seal any holes, gaps, or crevices to prevent area of harborage for vermin.

    One live but dying (on it's back) cockroach observed in front of the water heater. [CA] The premises of each food facility must be maintained free of vermin and vermin debris. [COS] Operator eliminated and removed cockroach. Photograph taken for documentation purposes. No other activity (live or dead), or additional evidence of an infestation was observed. Last two pest control service reports was reviewed. Pest control was provided by Terminix dated 5/13/25 and 6/10/25. No vermin activity was noted on either reports. A follow-up inspection will be conducted and any evidence of vermin may result in further enforcement action.
