    Observed multiple foods are stored in unapproved or single-use containers. [CA] Discontinue storage of food in unapproved containers. Food shall be stored in food grade containers approved for sanitation.

    Observed several broken floor tiles throughout the facility, especially in the warewashing area. Observed standing water in the gaps of the floor. [CA] Floors, walls, and ceilings shall be maintained clean and in good repair.

    Observed several gaps in the ceiling by the ice machine. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage.

Meeting with Javier Perez regarding the Los Chavitos INC commissary agreement. Commissary Approval.

    Bread was stored on bottom shelve below meat in side walk in freezer. [CA] Ready to eat food shall be stored on top shelve and chicken shall be on the bottom shelve. Proper refrigeration/freezer storage sticker was handed out.

    Employee don't know were test strips are stored. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

    Two out of two hand washing sinks were blocked. Hand washing sinks in the cooking area has bags in it and was blocked with table with rice maker. Other hand washing sink in the ware wash area is blocked with garbage bin and was in use for throwing vegetable trim. [CA] Hand washing facility shall be clean, unobstructed, and accessible at all times for employees to use. [COS] PIC made both sinks accessible. Hand washing sink in the cooking area lack paper towel. [CA] Properly wash hands with soap, warm water and dry using single use paper towels as required.

    Drain pipe from the ice machine is inside the drain. [CA] All equipment that discharges liquid waste shall be drained by means of indirect waste pipes, and all wastes drained by them shall discharge through an airgap into a floor sink or other approved type of receptor. Provide minimum of 1 inch airgap from drain pipe to floor rim.

    Observed black mold like debris at the steel panel inside the ice machine. [CA] Clean inside of the ice machine more frequently to maintain. Cardboard in use on floor inside the walk in refrigeration as a mat. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.
