[CA] Do not obstruct the handsink. Handsinks must be accessible so that handwashing is not only possible, but likely. [COS] The prep table was moved away.

    Sanitizer test strips were not available upon request. [CA] Obtain sanitizer test strips.

    Observed lack of approved cove-base flooring in the restroom. [CA] Provide approved cove-base flooring. Comply by 10/9/26. Recommend consulting Plan Check before installing new flooring.

    Observed 7 small electric rice cookers were on a countertop in the dining area, next to tables/chairs. ANSI/NSF certification could not be verified. [CA] All utensils and equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). Prior to the installation of any new equipment, submittal is required to Plan Check. Note: facility is permitted to use the NSF electric rice cooker (operators stated NSF rice cooker existed in the facility since the before the previous owner), so long as it is not accessible by customers and remains in the kitchen.     Observed the following at the only kitchen handwash sink: - An open water pitcher (for drinking water) was stored inside the sink, making the sink unusable. - A table with water pitchers on top was placed directly in front of the handwash sink, obstructing it. - The paper towel dispenser was empty; an uncovered roll of paper towels was stored nearby. [CA] Do not store items inside the handwash sink. Do not place items in front of the handwash sink. Paper towels must be stocked inside the dispenser at all times. [COS] The water pitcher was removed. The table was moved away. Napkins were placed inside the paper towel dispenser.

    Observed heavy accumulation of food debris/grease on floors under cooking equipment. [CA] Keep floors clean.

    The water at the only handwash sink was shut off at the start of the inspection. When an employee turned on the water, the water was brown for a second. Water continued to run clear afterwards. [CA] The handwash sink must be functional and usable at all times. Keep in good condition. Water must be clean and clear.

    Inside an upright refrigerator: observed open bowls of raw fish and raw shrimp stored above open container of lotus roots. [CA] Store raw meats below/away from ready-to-eat foods to prevent cross-contamination.

    Observed cardboard lining floors in kitchen. [CA] Discard cardboard. Surfaces must be smooth, durable and easily cleanable.

    When asked to wash his hands, employee rinsed his hands at the prep sink without taking off his gloves. [CA] When washing hands, discard gloves, wash hands at the handwash sink with soap, dry with paper towels, and put on new gloves.
