Major: 0 PPM chlorine measured inside the dishwasher during active warewashing. [CA] Maintain chlorine in mechanical warewash machine at 50 PPM. [SA] Employee set up a sanitizing solution (100ppm Chlorine) compartment in the 3-comp sink. Heavy mold-like substance buildup inside the ice machine. [CA] Clean and sanitize routinely.

Below the 3-comp sink and in the dry storage room : observed insecticide containers that are not approved for use in a food facility. [CA] Only those insecticides, rodenticides, and other pesticides that are necessary and specifically approved for use in a food facility may be used. Chemical container (lubricant) stored on shelves near dry food items. [CA] Store chemicals separate and away from the storage and preparation of food or food contact surfaces and linens.

Inside the glass door refrigerator in the dry storage room: measured the following PHFs at 45F: hard boiled eggs, yogurt, dressing to go cups. [CA] Cold PHFs shall be held at 41°F or below.
