- On-site for operator scheduled follow-up inspection after facility was closed due to a cockroach infestation on 10/25/2024 and again on 10/26/2024. - Facility is hereby authorized to reopen and resume operations. - Continue to clean and maintain facility free of vermin. - This Division recommends: - Increasing frequency of service to twice per month, especially during warmer months or when activity is observed. - Replacing cardboard boxes with plastic containers with tight fitting lids.

    One dead cockroach observed inside crevice of the right front wheel of the three-door upright freezer. Facility was serviced by licensed pest control service on 10/25/2024, 10/26/2024, and again on 10/27/2024. Pest control reports were provided for review. No other live or dead activity was observed. [CA] Clean and sanitize area of dead cockroaches or old droppings. The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. FOLLOW-UP INSPECTION

Note: This is a follow-up inspection to the limited inspection conducted yesterday by Lawrence Dodson. The facility was closed due to cockroach infestation. During this follow-up inspection, one live cockroach was observed below the steam table and another one on the wall above the to go box storage shelf. Therefore, the facility shall be remain closed. The owner shall be billed $645.00 for this after-hour follow-up inspection. Please conduct a pest control intervention, thorough cleaning, and sanitizing of the facility, and then contact our office at 408 918 3433 or 408 408 918 3451 to schedule a second follow-up inspection on Monday, 10/28/24. [CA] Eliminate all insects/rodents from the facility by a legal method. Until the live cockroaches are all eliminate and verified by a district inspector, the facility shall be closed. Priscilla Mark, a Supervising Registered Environmental Health Specialist with the county of Santa Clara has approved the closure.
