    Cheese kept at the prep surface under TPHC had one time marking indicating PHFs were kept for more than 4 hours, and a separate time marking indicating a different discard time. PIC stated the cheese was taken out of the walk in refrigerator less than 1 hour ago and employees did not remove the previous discard time. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review.

    The time sticker on the cheese at the sandwich assembly area read "use by 9:13 AM October 30, 2025". Per manager, the cheese was just taken out from refrigeration. Observed the time sticker to be the use by date and time when cheese is maintained in refrigeration. [Corrective Action] Ensure a new time sticker is printed out with a 4 hour discard time when cheese is removed from refrigeration. [Corrected on Site] Employee printed out new time sticker with a 4 hour discard time.

    Warm water at the automatic handwash sink in the kitchen and women's restroom was measured below 100 degrees Fahrenheit. [Corrective Action] Ensure warm water is maintained at 100 - 108 degrees Fahrenheit when hot and cold water is pre-mixed.

    Observed broken door at the Frappe machine. [Corrective Action] Repair the door.

    Cheese on table had time marker for 6:25PM at 2:30PM. Measured cheese at 64F. Container that cheese came from was marked with 1:33PM and 3:25PM stickers. Per employees, cheese is removed from cooler and time marked for 1 hour (time to put on table) and 4 hours (total time of use). Employees state that cheese is only used for a maximum of 4 hours once removed from the cooler. PIC states that time marker was incorrect. [CA] When time is used as a public health control (TPHC), PHFs shall be accurately time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Correct time marker of 3:25PM was placed for cheese on table.
