OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: DI VANG RESTAURANT NEW OWNER: STEVE G. SOLIDARIOS The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. Per application form and per discussion with owner via phone, this ownership change occurred in October 2024. A 25% penalty fee for operating without a permit will be applied. An invoice for the permit fee in the amount of $1,975.00 ($1,580.00 + $395.00 25% penalty fee) will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 09/01/2025 - 08/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 8/18/25 *Permit condition: N/A *Obtain food safety manager certificate within 60 days. Observed the menu offered "sunny-side up eggs" (on banh mi), but no consumer advisory was available. [CA] If a menu offers undercooked animal product, a consumer advisory must be provided on the menu. The consumer advisory consists of BOTH: 1) "Disclosure": Clearly indicate WHICH menu items contain undercooked animal products. AND 2) "Reminder": The following statement in writing on the menu: “Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions.”

Observed accumulation of food debris on multiple surfaces, including: refrigerator door handles, equipment (e.g. scale on dry storage shelf), on sides of juicer). [CA] Keep surfaces clean.

Inside the prep refrigerator's reach-under section: observed bowl of raw meat stored directly over noodles. Inside the upright freezer: observed a full beverage pitcher was stored among/underneath raw meats. [CA] Store raw meats/animal products below and away from ready-to-eat foods to prevent contamination.

A metal probe thermometer was not available upon request. [CA] Keep a metal probe thermometer readily available to measure internal food temperatures.

Observed heavy discoloration and dust accumulation on ceilings in kitchen. Observed food debris splatters/accumulation on walls. Observed heavy accumulation of food debris on floors, especially behind the prep unit and refrigerators. [CA] Keep floors, walls, and ceilings clean.

1) Observed aluminum foil was used to line shelves/surfaces. Observed soiled plastic wrap around small scale on dry storage shelf. [CA] Discard foil and plastic wrap and discontinue using it to line surfaces. All surfaces must be smooth, durable, and easily cleanable. 2) Observed round wooden cutting board was damaged, creating a crevice for food to accumulate. [CA] Discard damaged cutting board.
