ROUTINE INSPECTION

Facility closed down on 9/9/24 after a cockroach was found under the cappuccino machine during a complaint investigation. During the complaint follow up inspection on 9/13/24, did not see any live/dead cockroach. Facility will reopen tomorrow 9/14/24 because they are still cleaning up. Facility has a POLARIS PEST ELIMINATION service. 1-877-510-9766. Facility is still working on closing all crevices in wooden shelving. Facility shall replace broken/missing base cove tiles under the cappuccino machine. Facility shall close the cavity behind the cold holding unit under the cappuccino machine. Facility shall clean and sanitizer the floor, wall, equipment before opening.

Inspection conducted in conjunction to CO0155289 **Facility is hereby closed due to LIVE cockroaches infestation. Once the violation warranting the closure has been corrected, contact environmental health specialist Hina Wyne at 408/918-3475 or the main office at 408/918-3400 for a follow up inspection.** ***Facility shall cease and desist all sales. *** A follow up inspection shall be conducted. ***Subsequent follow up inspection after first follow up shall be billed $298/hr during business hours and $645/hrs (minimum of 2 hours) during nonbusiness hours upon availability.*** Consulted with Priscilla Mark.
