ROUTINE INSPECTION
Notes: - A follow up inspection will be provided to verify compliance with violations noted. - Facility lacks a janitorial sink - The handwash station has two separate faucets for hot and cold water. A single faucet may be required if water at the hot water faucet is above 108F and not readily adjustable.
The floors of the walk in cooler and walk in freezer have an accumulation of food debris or debris. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.
There are prepackaged food items lacking labels. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor. [COS] PIC provided the labels of the food products.
1. Second Repeat Violation: Heavy accumulation of build up/dust observed on toilet and handwash station. [CA] Toilet facilities shall remain clean and in good repair. Routinely clean and sanitize. 2. Second Repeat Violation: Boxes and freezer are blocking hallway to restroom. [CA] Ensure the restroom is accessible to facility employees at all times. Allow for at least 30 inches of space to pass through. 3. No toilet tissue was available in the restroom. [CA] Toilet tissue shall be provided in a permanently installed dispenser at each toilet.
1. The walk in cooler was not maintaining PHFs at 41F or below. [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below. 2. Facility is using cardboard to line the floors of the facility. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.
Food is stored on the floor of the walk in freezer. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.
1. The restroom handwash station lacks a handwash reminder sign. [CA] A sign or poster that notifies food employees to wash their hands shall be posted at all handwashing stations used by food employees. 2. Facility lacks the most recent health inspection report. [CA] A copy of the most recent routine inspection report conducted to assess compliance with this part shall be maintained at the food facility and made available upon request.
Facility has several pieces of equipment not required for the operation stored in the hallway between the sales floor and the restrooms. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.
PHFs in the walk in cooler were measured between 47-48F. Per PIC, they changed the temperature of the walk in cooler yesterday as it had frozen over. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Items were discarded. See VC&D report.
FOLLOW-UP INSPECTION
This is a follow-up inspection to the routine inspection conducted on 11/20/24. K30: Operator purchased dunnage racks for walk in cooler and walk in freezer. All food is being stored 6 inches above the floors. K35: Majority of the cardboard has been removed from the facility. Some cardboard observed in walk in freezer (stuck to blocks of ice). Operator stated that he will be melting the blocks of ice and removing the remaining cardboard. Pictures will be sent to district specialist within 2 weeks to verify compliance. K43: Toilet facility is still an issue. Per operator, he will be hiring someone to help him clean/sanitize restroom. Pictures will be sent to district specialist within 2 weeks to verify compliance.
ROUTINE INSPECTION
A follow up inspection will be conducted within 3 business days. The 1st follow-up inspection is free of charge. Any and all subsequent follow-up inspections will be billed at the approved hourly rate of $298/hr, minimum of one hour.

REPEAT VIOLATION: 1) Cardboard is being used to line walk in cooler floor, walk in freezer floor, and shelves/racks throughout entire facility. [CA] Discontinue use of unapproved materials. 2) Two freezer units in hallway leading to bathroom were observed inoperable. [CA] Remove inoperable equipment from premises to prevent pest harborage.
Heavy accumulation of dust, dirt, and other debris observed in walk in cooler floor, restroom floor, and floors throughout facility. [CA] Routinely clean and sanitize floors to prevent pest harborage.
