ROUTINE INSPECTION
Facility is hereby closed due to evidence of a vermin infestation. Facility is to immediately cease and desist all operations. Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. [CA] Food is adulterated if it bears or contains any poisonous or deleterious substance that may render it impure or injurious to health and shall be discarded immediately.[COS] PIC voluntarily condemned and destroyed 2 bottles of liquor containing flies.

Lack of single use paper towels at hand wash sink. [CA] [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels.
Spoons used to mix drinks are stored in the vicinity of the hand wash sink.[CA] All cleaned utensils shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least six inches above the floor.
Drain pipe for ice bin extends into floor sink. [CA] The potable water supply shall be protected with a backflow or back siphonage protection device.
