    The only handwash station at the cooking area was blocked off by a portable AC unit. [CA] Ensure the handwash station is accessible at all times. [COS] Staff relocated the A/C unit to unobstruct the handwash station.

    1. A dead cockroach was observed on the wall near the water heater. 2. A hatched cockroach egg sack (ootheca) was observed on the wall in the staff room. [CA] Clean and sanitize area of dead cockroaches or old droppings. Provide pest control as necessary.

    1. The restrooms lack handwash reminder signs for staff. [COS] The open box of to go bags was discarded. Other items were moved inside.

FOLLOW-UP INSPECTION

This is the first follow up inspection for the routine inspection where the facility was closed for an active sewage back up. This is also an after business hour inspection and is subject to a $645 dollar charge. Observation: K22: The mop sink was cleaned out by a plumber and is draining properly. Use of the three compartment sink does not cause the mop sink to back up. Ensure to throughly clean and sanitize affected area. Facility is okay to reopen.

This is a follow up inspection for the routine inspection conducted on 01/09/25. Observations: K06: All hand wash stations are fully stocked. K23: No live or dead cockroaches were observed. Facility was treated by EcoLab last night at 11:00 pm. Continue to provide pest control as necessary. Facility okay to open. Continue to correct any pending violations.

[CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] PIC changed the batteries in the dispenser at time of the inspection. 2. In the drive through area, at the hand sink inbetween the drive through window and ware washing area, the soap dispenser was in disrepair. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. [COS] PIC changed the hand soap dispenser from a battery operated one to a push one at time of the inspection.

    1. In the kitchen area, on the floor behind the undercounter pie oven, a nymph was observed on a fry. 2. In the front of house area, near the frappe and milkshake machine, observed two adult cockroaches and a nymph. 3. In the dining area, on the ceiling above the self-serve soda dispensers an accumulation of cockroach droppings were observed on the ceiling tiles. 4. An accumulation of cockroach droppings were observed on the ceiling tiles above the lockers. Note: Facility receives monthly pest control from EcoLab. Pest control reports dated on 11/27/24 and 12/24/24 both indicate no cockroach activity and treatment with both bait and crack and crevice pesticide. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. Small flies are present throughout the facility in the dining room, restroom, kitchen area, front bar area. No direct contamination of food contact surfaces were observed.     Facility is using Time as a Public Health Control for burger assembly ingredients (tomatoes, cheese, lettuce, etc) and observed the following: 1. Food items were not time marked. 2. Food items were being used past their time mark for discard. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] All items were discarded as facility was closed. See K23 for more details on closure.
