    Observed drainpipes from the 1-door preparation unit (front) and the warewashing area (kitchen) extend directly into the floor sink. [CA] Provide an air gap between the bottom of the drainpipe and the rim of the floor sink at least twice the diameter of the inlet and no less than one inch.

Per employee, food was moved from the walk-in cooler and relocated to the warmer less than 2 hours prior to temperature measurement and is finished reheating. [CA] Potentially Hazardous Foods that have been cooked, cooled, and reheated for hot holding shall be reheated to a minimum internal temperature of 165°F for 15 seconds. [COS] Specialist directed employee to immediately reheat the curry on the stovetop to 165F prior to hot holding.

FOLLOW-UP INSPECTION

On site for 2nd follow up inspection. Facility was closed due to cockroach infestation. 1 dead old cockroach observed under empty bucket containers in front of hand wash sink. No other dead or live activity observed in the facility. Facility has cleaned floors and equipment. Per PIC, some racks are being removed and new ones will be installed. Cracked floors has been fixed. Gaps between warewash sink, prep sink and wall have been sealed. Continue to clean cook line area and maintain items 6 inches above the floor. Continue to obtain professional pest control services as required. Pest control report from 11/21/25 reviewed. Facility is ok to reopen.

FOLLOW-UP INSPECTION

On-site to conduct a follow-up inspection to the facility closure (11/18/2025). The facility will remain closed due to continued evidence of vermin infestation (see violation K23). The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. - Reviewed pest control reports from Dewey Pest Control Co. Per pest control services report on 11/18/2025, the facility kitchen and front dining area was fogged for cockroaches. Per pest control services report on 11/19/2025, no live activity was observed. - At the time of inspection, facility was conducting a thorough cleaning of the food preparation area, storage area, and cook line equipment. Facility is in the process of sealing gaps between the wall and the warewashing area, repairing floor tiles, and replacing the grout. Complete cleaning prior to re-inspection. - This follow-up inspection is free of charge. The second follow-up inspection will be charged, and payment must be received prior to re-inspection. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour (one hour minimum) during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 (minimum of two hours) during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Limited inspection conducted in conjunction with CO0158361. The facility is hereby CLOSED by this department due to evidence of vermin infestation (see violation K23). The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Provide warmer with adjustable temperature control to keep PHFs at 135F and above instead of intermittently turning on and off warmer. [COS] Operator turned on warmer and shall keep warmer on at all times.
