Paper towel dispenser in the restrooms were observed broken and unable to dispense paper towels. Loose roll of paper towels were observed stored on top of the dispenser. [Corrective Action] Repair or replace the paper towel dispenser in the restrooms.

Observed toilet paper stored in a basket next to the toilet in the restrooms. [Corrective Action] Store toilet paper in the toilet paper dispenser.

1. Lack of paper towels in the restroom. [Corrective Action] Ensure paper towels are available to dry hands at all times. 2. Soap dispenser observed not mounted to the wall. [Corrective Action] Mount the soap dispenser to the wall.

Helium cylinders at the back area observed not secured to a rigid structure. [Corrective Action] Secure the helium tanks to a rigid structure.
