    On the counter-top, observed a container of sliced white cheese stored with a TPHC label for two minutes prior, and measured at 69F. Per operator, was not able to determine the time the sliced white cheese was removed from temperature control. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Operator discarded container of sliced white cheese.

    Observed a detached cove base tile on the floor directly to the left of the ice machine. Observed accumulation of debris in the area below the ice machine. In the dry storage area, observed liquid accumulation on the floor below shelving. [CA] Ensure flooring is properly repaired and any gaps sealed to prevent possible harborage of vermin. Floor shall be frequently cleaned and maintained to prevent harborage of vermin.

    Observed several fruit flies in the facility. [CA] Ensure facility is kept free of fruit flies to prevent possible contamination of food.
