FP11 $1,580.00 will be invoiced and mailed to the address on the application. Payment shall be paid within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received within 30 days of the change of owner’s date. Permit issued and valid: 09/01/2025 08/31/26-. The permit will be deemed invalid if the permit fee is not paid in full within 30 days of the date of this report. This report serves as a temporary permit until the official permit is mailed to the facility. When the invoice for the facility is received, payment must be made within 10 days. FOLLOW-UP INSPECTION

Note: This is a second follow-up inspection to the routine inspection that was conducted on 07/31/2025. The facility was closed for vermin infestation. The owner hired two pest control companies and conducted multiple interventions. The booth sits were all removed and the dining area cleaned. Most of the structural deficiencies are also addressed. Checklist for reopening after closure and couple of pest control company report are collected. During today's follow-up inspection, no live or dead cockroaches were observed in the restaurant. Therefore, the restaurant is open for service.

FOLLOW-UP INSPECTION

Note: This is a follow-up inspection to the routine inspection conducted on 07/31/2025. The facility was closed due to vermin infestation. The owner hired a pest control company and conducted intervention, conducted cleaning, send our office completed reopening checklist, and requested an inspection for today. During today's follow-up inspection, many live and dead cockroaches were observed on the same location where I found live once last Thursday. Many live and dead cockroaches were also observed on dining area below the sitting booth. Therefore, the facility is going to remain closed. This first follow-up inspection is a free inspection, however, any subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for the first 2 hours, thereafter $298 each additional hour, during non-business hours, and upon inspector availability The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered up, relocated, tampered, or copied.
