  • K45: Floor, walls, ceilings: built,maintained, clean

    Observed coved base tiles directly below the three-compartment sink sunken into the wall and creating gaps and/or crevices for vermin harborage. [CA] The juncture of the floor and wall shall be coved with a 3/8-inch minimum radius coving and shall extend up the wall at least 4 inches.

  • K16: Compliance with shell stock tags, condition, display

    Observed shellstock tags stored inside clear plastic baskets on the side of the three-door upright refrigerator commingled together. [CA] The identity of the source of shellstock that are sold or served shall be maintained for 90 calendar days from the dates of harvest by using an approved recordkeeping system that keeps the tags or labels in chronological order correlated to the date or dates the shellstock are sold or served.

  • K27: Food separated and protected

    Raw shelled eggs maintained above ready-to-eat food items inside the walk-in refrigerator. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

  • K23: No rodents, insects, birds, or animals

    Two dead German cockroaches observed on the floor by the rear door by the walk-in refrigerator. One dead cockroach observed inside sticky monitoring trap placed above the water filter by the espresso machine. Per manager, the facility noticed activity and increased service to once a week. Last serviced was conducted on 4/28/2025. Pest control report was provided for review. Manager stated that next service is supposed to be conducted the following week and will return to monthly services. No other activity was observed throughout the facility. [CA] Clean and sanitize area of dead cockroaches.

  • K07: Proper hot and cold holding temperatures

    Observed pan of cooked vegetables maintained on top of the steam tables measuring at 97F. Manager stated that vegetables were just blanched and sautéed approximately 5 minutes prior and is in preparation for orders. At the time, no orders were placed. [CA] PHFs shall be held at 41°F or below or at 135°F or above.
