ROUTINE INSPECTION
--48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $1,383.75. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. A copy of the invoice was provided to Owner during inspection. Proper refrigeration storage sticker provided.

Prep sink spigot faucet observed leaking due to holes/gaps. [CA]: Repair spigot faucet and maintain in good condition.

Missing Quat Ammonium sanitizer testing method. [CA]: Provide Quat Ammonium testing method (for ex: test strips). Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds.

Missing food handler training for employees. [CA]: All employees that handle open food or food contact surfaces shall obtain valid Food Handler Cards within 30 days of employment. Food Handler Cards shall be maintained on site and available for review at all times.

Expired health permit. [CA]: Facility shall always operate with a valid health permit. 48 hour notice issued to renew health permit. Failure to comply may be subject to enforcement actions including facility closure.

At the restroom, single use towel dispenser case observed flashing red and was not dispensing single use towels. Single use towels provided inside. [CA]: Hand sink stations shall be equipped with single use towels and soap at all times. Dispensers shall be operable at all times. [SA]: Single use towels provided outside the dispenser and owner will replace battery today.

*REPEAT VIOLATION* Uncovered bulk containers of dry food in back prep area. [CA]: Ensure bulk containers of dry food are stored in food grade containers and covered with a fitted lid.

At the front prep line cooler reach in, raw lamb was stored above ready to eat produce. At the walk in cooler, raw shell eggs stored above feta cheese and sauce containers. [CA]: Properly store raw meat beneath and separate from ready to eat food to avoid potential contamination.
