OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: BitterBuck NEW OWNER: BitterBuck 408, LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 06/01/25 - 05/31/26 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 05/08/25 *Permit condition: 1. Seal the acrylic panels in the restrooms. 2. Menu is limited to cold foods until the ovens are approved. 3. Submit plans for the addition of the Merry Chef and the tentative iCombi ovens by 5/16/2025. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.

The acrylic wall panels in the restrooms are not sealed. [CA] The walls / ceilings shall have durable, smooth, nonabsorbent, light-colored, and washable surfaces. Seal the panels to make them washable.

Hot water throughout the facility measured at a maximum of 117F. [CA] An adequate, protected, pressurized, potable supply of hot water and cold water shall be provided at all times. Hot water shall measure at 120F minimum.

Both of the restroom doors have the self closing device in disrepair. [CA] Toilet rooms shall be separated by a well-fitting self-closing door. Repair or replace the self closing devices.

Facility has added a Merry Chef oven without going through plan check. [CA] A person proposing to build or remodel a food facility shall submit plans for approval before starting any new construction or remodeling of any facility for use as a retail food facility.
