Completed a reinspection with the owner's. Observed a dead cockroach on the floor in the kitchen. The facility must remain closed. Clean up and take effective measures to eliminate the roaches. Call for a reinspection and be sure there are no live or dead roaches in order to reopen. The reinspection will be a charged inspection. The past due permit fee has been paid for and the receipt shown during this inspection.

Limited inspection conducted in conjunction with CO0158025 FACILITY IS CLOSED DUE TO A COCKROACH INFESTATION The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. 3. Environmental Health Permit expired on 8/31/25; complete payment for overdue permit invoice and submit receipt of payment. Once the above is corrected, submitted the completed items and related documents to district specialist. Follow-up for routine inspection conducted on 7/9/25. Verified that major violations are still in compliance: - Handwash sinks are operable with water, soap, and single-use paper towels. - Food contact surfaces/equipment are cleaned and sanitized. - Hot water is available at the three compartment sink and measured at 120F. The following items were also observed at time of inspection: - Did not observed signs of vermin at time of inspection. Reviewed pest control service report dated 7/11/25 that marked treatment was done but no additional details. - Unapproved propane cook top was removed from the cook line and propane tanks removed from facility. The cook top was placed in the storage room. Per operator, the cook top is used at temporary events or catering. -

All major violations observed to still be in compliance. - All handwash sink have warm water, soap, and single-use paper towels available. - Potentially hazardous foods inside the kitchen are observed and measured at proper refrigeration storage.
