ROUTINE INSPECTION

1. Observed Vermin: Documented in the following areas: Fresh rodent droppings behind keg refrigerators in main bar area, behind sinks and equipment in side bar area, and in the dry storage area under and on racks and bulk bins. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Jessica Zertuche 4. Notification: The person in charge during inspection, Laura, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

Thoroughly clean and sanitize entire facility including bulk bins and cans with a dilute bleach solution of 100ppm.

    Drainage pipe from ice machine was being emptied into a full bucket of water located in the mop sink. Pipe was submerged in the water. [CA] All equipment that discharges liquid waste shall be drained by means of indirect waste pipes, and all wastes drained by them shall discharge through a minimum 1" airgap into a floor sink or other approved type of receptor.

    Ceiling panels in kitchen area are in poor condition (covered in dust and large gaps in between). Observed broken/missing tiles in several areas. Observed large gaps between wall and ceiling. [CA] Clean ceiling panels and minimize gaps (replace with approved cleanable panels if necessary). Replace cracked missing floor tiles. Seal gaps.
