    MAJOR: 1) Tamales on front counter measured to be 93F - 121F. Per employee tamales were pulled from stove about 2.5 hours and stored on countertop covered with plastic bag. Per operator, they intend to place tamales in cooler to get them cold later. [CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. [COS] Tamales will be kept until 2:30PM (up to 4 hours) and then discarded if not used. Facility may not cool down these tamales for later use. MINOR: 2) Chicken soup (117F), stock (112F), and tamales (167F) observed to be in large pots in the back of facility. Tamales observed to be covered by plastic bag. Per operator the items were cooked at the following times: -Chicken soup at 1.5 hours prior -Stock at 1.5 hours prior -Tamales at 10 minutes prior. [CA] When cooling keep PHFs uncovered/loosely covered in small containers to rapidly cool PHFs. Facilities may use any combination of cooling methods listed above. [COS]Chicken was removed from soup and placed directly in freezer on tray. Stock and soup were placed into ice bath to rapidly cool down. Tamales were uncovered and will be transfered to tray prior to being placed inside walk in to cooler.

[CA] Provide scoops with handlles and store scoops such that handles do not make contact with the food.

Ensure hand soap and paper towels are readily available and easily accessible at all times. [COS] Owner refilled towel dispenser. Paper towels were maintained outside of dispensers at all restroom hand wash stations. [CA] Paper towels shall be stored inside the dispenser to prevent potential contamination.

- On-site for follow-up inspection after facility earned a conditional pass during a routine inspection on 9/30/2024. - Facility has corrected the following: - K05M - hands not clean/improperly washed/gloves not used properly: - At time of inspection, employees observed properly washing hands as required. - K09M - Improper cooling methods: - Observed properly cooling methods being utilized. At time of inspection, ice paddle was being used to cool cooked beans. - According to cooling time and temperature measured, time and temperature requirements were met. - Reminder: Properly cool PHF items from 135F to 70F and below within 2 hours, and from 70F to 41F and below within an additional 4 hours. - K13M - Food not in food condition/unsafe/adulterated: - Food appeared in good and safe condition. - Observed no adulteration and/or contamination of food. - K14M - Food contact surfaces unclean and unsanitized: - At time of inspection, active ware washing was not observed. - Sanitizing bucket at the preparation line was observed and measured within requirements. - The following violation was still observed: - K07M - Improper hot and cold holding temperatures. - See violation above. - Facility has earned a green pass placard. - Continue to address all other violations noted on inspection reports.

    Sliced tomatoes measured at the food preparation line in an inadeuqate ice bath with ice only touching the bottom of the container. Tomatoes were sliced within the last two hours and measured at 67F. * Discussed with PIC removing non-PHF items from ice bin to provide adequate spacing and/or providing large ice bin. Tamales maintained at the cook line measured at 109F. PIC stated that tamales were steamed approximately two hours prior. * Discussed the usage of time as a public health control for tamales or cooling PHF item to be reheated for immediate service. Numerous PHF items maintained inside the two-door upright refrigerator measured between 43F to 48F. Thermostat above the the refrigerator display a temperature of 56F and was slowing decreasing. Observed employees constantly opening and closing refrigerator to obtain food for preparation. Probe thermometer placed inside the unit initially measured at temperature of 53F, and then eventually 51F after manager had decreased the temperature. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Additional ice was added into ice bath with certain non-PHF items removed. Tamales will be served and subjected to TPHC.

    Ambient temperature of the two-door upright refrigerator measured initially at 53F and then at 51F after manager decreased the temperature of the unit. [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below. Discontinue storing PHF items inside the refrigerator and continue to monitor unit. Unit may be used to storage PHF items once it is able to maintain PHF at 41F or below. This Division suggests maintaining refrigerator at temperature lower than 40F.

At time of inspection, rice was been cooling for 4 hours. [CA] Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F pr below within 2 hours and from 70°F to 41°F or below within an additional 4 hours. Numerous PHF items (raw chicken, cooked chicken, raw shelled eggs, various soups, numerous bags of cheeses, etc.) maintained inside the two-door upright refrigerator in the main kitchen measured between 47F to 55F. Items were either maintained inside the refrigerator or were recently used for preparation. Ambient temperature of the refrigerator measured at 54F via probe thermometer placed inside. Numerous pots of soup maintained below the cook line measured between 100F to 130F and were used for preparation less than one hour prior. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Ice was added to ice bath to immediately cool tomatoes. All PHF items maintained inside the two-door upright refrigerator were relocated to separate refrigerators to immediately cool. Pots of soup below the cook line were reheated to hot hold.

(2) After touching bare human body parts other than clean hands and clean, exposed portions of arms. (3) Immediately after using the toilet room and again when returning into the kitchen. (5) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking. (6) After handling soiled equipment or utensils. (7) During food preparation as often as necessary to remove soil and contamination and to prevent cross-contamination when changing tasks. (8) When switching between working with raw food and working with ready-to-eat foods. (9) Before initially donning gloves for working with food.
