The permit category for this facility is FP11. An invoice for the permit fee in the amount of $2,281.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 04/01/2026 - 03/31/2027 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. Observed restroom converted into office space with what appears to be sleeping arrangement. Toilet has been removed, however, hand sink is still available for use with supply of hot and cold water. [CA] Restroom may not be converted to an office, resulting in a toilet facility that does not meet local building and plumbing ordinances.

Discoloration and black mold-like growth matter observed on walls above the three-compartment sink. [CA] Walls and/or floors in preparation area shall be kept clean.

Accumulation of green growth-like matter inside the hand wash sink wash basin at the sushi bar. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times.

Water heater installed directly on the ground. [CA] Water heater shall be installed on stand or curb. All utensils and equipment shall be approved, installed properly, & meet applicable standards.
