FACILITY IS CLOSED DUE TO A COCKROACH INFESTATION The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. *The facility is required to CEASE OPERATIONS IMMEDIATELY and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. FOLLOW-UP INSPECTION

Follow-up inspection for facility that was closed on 1/14/25 for cockroach infestation. Facility received pest control treatment on 1/15/25. Reviewed service report that notes treatment was done at areas of concern including cracks and crevices. Service and treatment will continue weekly until the end of the month and will then move to a monthly service. At time of inspection, one live but slow moving cockroach was observed on shelf above the food prep sink. Cockroach was killed and removed by operator. Did not observed any other evidence of cockroaches in the facility. All areas in the kitchen has been cleaned and sanitized. Gaps and crevices in areas where activity was previously observed has been sealed. Facility is okay to reopen This is a subsequent follow-up inspection and will be billed $298.

Follow-up for routine inspection conducted on 1/14/25 for facility closed due to cockroach infestation. Live cockroaches were observed during the inspection - Four live cockroaches, adults and nymphs, in crevice under the food preparation sink. - One adult cockroach and three nymphs observed behind shelf above the three compartment sink. Facility had an initial inspection with Clark Pest Control on 1/15/25. Service report notes evidence of German Cockroaches and a clean out is recommended. Facility shall remain closed until there are no evidence of a cockroach infestation. The subsequent follow-up inspection will be charge $298/hour, minimum one hour, during normal business hours (Mon - Fri 7:30am - 4:30pm), and $645/hour, minimum two hours, during non business hours and subject to availability of an inspector.
