Raw chicken observed thawing in standing water in the right-side compartment of the two compartment preparation sink. Shrimp observed thawing in standing water in a container in the three compartment dishwashing sink. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

[CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan.     1. Dishwashing employee observed spraying down dirty dishes using a sprayer hose at the dishwashing area. Backsplash from spray observed landing on debris encrusted metal backboard situated behind the sink. A steady stream of backsplash water observed dripping off of the soiled/debris encrusted metal backboard directly into/on top of basil in an open case that was maintained on top of two soy sauce buckets. [CA] Food is adulterated if it bears or contains any poisonous or deleterious substance that may render it impure or injurious to health and shall be discarded immediately. [COS] Basil was voluntarily discarded into the trash. 2. Employee observed turning on the cold water for copious amounts of raw chicken (60F) "thawing" while maintained in standing water in the right side compartment of the preparation sink. Employee proceeded to chop up a slab of raw beef and tossed it into the left-side compartment of the two-compartment preparation sink. Water observed overflowing from the right-side of the sink with the raw chicken onto the piece of raw beef maintained in the left-side compartment. [CA] Food shall be separated and protected from cross contamination. [SA] Beef was chopped up and cooked to greater than 165F.

Raw shrimp measured 62F while maintained in a tub in standing water in the three compartment dishwashing sink. Per staff, it had been taken out of the walk-in cooler to thaw less than 1 hour prior. Raw chicken measured 60F while maintained in standing water in the right-side compartment of the preparation sink at the rear of the kitchen. Per the cook, he had been thawing it for less than 2 hours. [Minor thawing to Major holding violation] Multiple potentially hazardous foods, including chicken broth, pooled egg, and cooked pork measured between 54F and 55F while maintained in the sliding, glass-door cooler at the entrance to the front service area. Ambient of the unit measured 54F via probe. Per staff, items had been placed into the unit earlier this morning, approximately 2 hours prior. Fried tofu measured 72F while maintained out of temperature control on top of a prep table across from the cook-line. Per staff, it had been fried less than 1.5 hours prior. Numerous par cooked and roasted ducks measured between 72F and 96F while hanging on hooks from the ceiling and/or from a cart at the back of the facility. Per staff, items had been par cooked or roasted less than 2 hours prior. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. Ensure that after cooking, potentially hazardous foods are immediately relocated to refrigeration to facilitate proper cooling/holding temperatures. [COS] Beef was prepared and cooked. Raw shrimp was relocated back inside the walk-in cooler. Raw chicken was placed into a pot of boiling water on the stove. Potentially hazardous foods in the upright sliding glass door cooler relocated to the walk-in cooler. Fried tofu relocated to inside prep cooler. Par cooked and roasted ducks relocated to the walk-in cooler.

Once bags of flour, rice, sugar, or other bulk substance is opened, they may no longer be stored in the dry storage room. Food shall be stored in NSF containers with tight fitting lids and maintained closed to prevent contamination. Food shall be stored a minimum of 6 inches off the floor.

On-site for a follow-up inspection after facility earned a conditional pass during the last limited inspection conducted on 2/12/2026 Compliance of the following has been verified: K05: Hands not clean/improperly washed: - Proper handwashing and glove-use procedures observed K07: Improper hot and cold holding temperatures - No potentially hazardous foods maintained out of temperature control, except for those actively being prepared or those subject to TPHC *Note: Numerous raw ducks and raw racks of pork measured between 51F and 52F while maintained on hooks at the back kitchen. Per staff, they had been taken out to dry prior to being cooked. Ensure that raw ducks and raw pork belly are not dried for more than 2 cumulative hours (of active preparation) before the cooking process. Ensure that after ducks and pork belly are cooked, one of the following three options is carried out: items are subjected to TPHC, immediately served, or placed into the walk-in cooler to immediately cool down. K08: Improperly using time as a public health control - All items subject to time as a public health control (pork, duck, chicken, etc.) were properly time logged at the front counter *Note: 3 racks of roast pork measured 136F while maintained on hooks at the back kitchen. Per staff, they were letting it rest for 10 minutes until being taken to the front counter to be hung up in the display case. Ensure that TPHC logs are immediately filled out for the items as soon as they are removed from the oven. *Note: Employee working at the duck preparation station had accurate knowledge of handling, cooking, and holding practices. This was a follow-up to the 8/18/25 routine inspection. Today's observations regarding the major violations: - K06: Observed all kitchen handsinks' paper towel and soap dispensers were stocked and functional. - K07: Observed one major temperature violation (bean sprouts); corrected on site. Note: Observed duck and pork hanging inside the walk-in refrigerator. Operator stated they had been left out for one hour to dry, then were placed inside the walk-in refrigerator 20 minutes ago. - K08: Observed meats at the front area were time-marked on the log. Note: operators stated they do not have any clams today because they did not receive the shipment yet. Due to the presence of just one major violation, the yellow placard was replaced with a green placard. OK to operate.

ROUTINE INSPECTION

Two or more major violations were cited during the routine inspection. A follow-up inspection will be conducted in the next 1 to 3 business days to verify compliance with all of the cited major violations. Subsequent follow-up inspections after the first shall be billed at $298/hour, one hour minimum, during normal business hours (7:30 AM to 4:30 PM), or $645 for a minimum of 2 hours, during non-business hours and in accordance with the inspector's availability. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered with, camouflaged, or copied. Failure to comply may result in a fine and enforcement action as per county ordinance section B11-55. Notes: Employee in charge of dishwashing states that they do not use sanitizer (bleach) when washing utensils or equipment that are too large to fit into the mechanical dishwasher (e.g. large pots), but rather only wash with soap and water. Manual dish washing shall be accomplished via wash, rinse, sanitize (minimum 30 seconds immersion in 100 ppm chlorine, i.e. bleach) in the 3 compartment sink, followed by air drying.   • K38: Adequate ventilation/lighting; designated areas, use

    Hood screens directly above the cook-line observed to have large amounts of grease buildup. [CA] Clean and maintain hood screens to ensure adequate ventilation of heat/grease.

  • K36: Equipment, utensils, linens: Proper storage and use

    1. Two knives observed to be stored in the crack between the counter and the prep unit across from the cook-line. [CA] Clean equipment and utensils that contact food shall be stored in a sanitary location. 2. Clean utensils observed to be stored uncovered underneath the paper towel dispenser next to the hand wash sink behind the front barbecue display unit. [CA] Clean equipment and utensils shall be stored elsewhere to preclude splash contamination of utensils from hand washing.

  • K01: Demonstration of knowledge; food safety certification

    *REPEAT VIOLATION Facility lacks a valid food safety certificate. All employee food handler cards could not be provided upon request. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

  • K30: Food storage: food storage containers identified

    1. Open containers of flour, rice, and other bulk food substances observed to be stored in the dry storage room. [CA] Per the facility's permitting conditions, ONLY the storage of prepackaged, non potentially hazardous foods is allowed in the dry storage room owing to its unapproved flooring and lack of cove-base. Once bags of flour, rice, sugar, or other bulk substance is opened, THEY MAY NOT LONGER BE STORED IN THE DRY STORAGE ROOM. 2. Containers of thawing chicken located directly on the floor in the walk-in cooler. [CA] Food shall be stored a minimum of 6 inches off the floor.

  • K07: Proper hot and cold holding temperaturesCritical

    The following potentially hazardous food measured in the temperature danger zone (greater than 41F, but less than 135F): - 6 racks of par-cooked roast pork (71F) hanging on hooks from the ceiling, directly inside the back door. Per staff, roast pork had been taken out of the walk-in to dry yesterday. - 14 cantonese style ducks (72F) hanging on hooks from the ceiling, directly inside the back door. Per staff, ducks had been taken out of the walk-in to dry 2 hours prior. - Raw beef (64F-67F) in large plastic tubs on a cart near the back door. Per employees, it had been taken out of the walk-in to thaw. See K26: Proper thawing procedures [minor thawing leading to a major holding violation] - 4 racks of par-cooked roast pork (68F-71F) hanging on hooks at eye-level, directly inside the back door. Per staff, they had been taken out of the walk-in to dry 2 hours prior. - Bean sprouts (64F) and sliced tomato (74F) maintained in containers on or above the counter across from the cook-line. Per staff, bean sprouts had been taken out of the walk-in 20 minutes prior, and tomatoes were cut less than 2 hours prior. - Marinated chicken (47F-52F) in the glass, one-door upright cooler behind the front barbecue display case. Per staff, chicken had been placed into the unit less than 1 hour prior. Ambient of the unit measured 50F. [CA] Potentially hazardous foods (PHF) shall be held at or below 41F, OR at or above 135F to prevent the growth of potentially harmful bacteria. *NOTE: Equipment MUST be capable of maintaining food at or below 41F, regardless of how often the unit is opened OR how warm the ambient air temperature outside is. Recommend calling a fridge technician for the one-door glass display cooler. [COS] Racks of roast pork VC&D. See VC&D report. Roasted ducks relocated to walk-in. Raw beef chopped up/prepped by the cook. 4 racks of roast pork relocated to walk-in. Bean sprouts and sliced tomato relocated to walk-in. Marinated chicken to be used or discarded within 4 hours of initially being moved to the unit. ____________ Minor violation: 1. Various raw meats (beef, pork, scallops) and cooked shrimp measured 45F-46F in the upper inserts of the large prep unit across from the cook-line. Per staff, all PHF was taken out of the walk-in and placed into the inserts less than 1 hour prior. Ambient of the lower part of the unit measured 41F. 2. Raw chicken measured 49F in the upper prep insert of the small prep unit across from the cook-line. Raw beef measured 46F in the lower part of the small prep unit. Ambient of the lower part of the unit measured 46F. Per staff, all PHF in the unit (upper and lower) had been placed inside less than 1 hour prior. [CA] Potentially hazardous foods (PHF) shall be held at or below 41F, OR at or above 135F to prevent the growth of potentially harmful bacteria. *NOTE: Equipment MUST be capable of maintaining food at or below 41F, regardless of how often the unit is opened OR how warm the ambient air temperature outside is. Recommend calling a fridge technician for both prep units (upper inserts of the larger prep unit, and both upper and lower part of the smaller prep unit). [COS] PHF in the larger unit (upper prep inserts) to be used or discarded within 4 hours of initially being taken out of the walk-in. All PHF in the smaller prep unit relocated to the walk-in for rapid cooling.

  • K05: Hands clean, properly washed; gloves used properly

    Employee observed handling raw chicken with bare hands, than picked up a knife and prepared to cut/handle ready-to-eat roast pork. Employee was instructed to wash his hands. [CA] Employees shall wash their hands after touching raw meat, prior to handling ready-to-eat foods. [COS] Employee washed properly washed his hands after being instructed to do so.

  • K06: Adequate handwash facilities supplied, accessibleCritical

    Hand wash sink in the front area with the meat display lacked soap in its wall-mounted dispenser. Both hand wash sinks in the kitchen lacked functional paper towels dispensers. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [SA] Soap and paper towels provided. ____________ Minor violation: Hand wash sink in the front area with the meat display and hand wash sink in the front area under to-go bags observed to be filled with dishes/various objects. [CA] The hand wash sink shall be used for no other purpose than hand washing. Keep the hand wash sinks unobstructed and available for use at all times that the facility is in operation.

  • K08: Time as a public health control; procedures & recordsCritical

    Roast duck, roast pork, and chicken in the front barbecue display case and/or in the steam table beneath AND a rack of roast pork maintained on the counter are on TPHC, per employees. Time log for roast duck observed to be filled out. Time logs for other items (pork, chicken) not filled out. [CA] When time only, rather than time and temperature is used as a public health control, ALL PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review.   • K04: Proper eating, tasting, drinking, tobacco use

    Employee in charge of chopping/preparing meat in the back observed drinking from an open bottle. [CA] When in food prep/food storage areas, employees shall drink from containers with tight fitting lids and straws to prevent contamination.

  • K16: Compliance with shell stock tags, condition, display

    Per staff and per menu review, facility maintains live clams for cooking and sale. Staff states that they ran out of clams last night. Shellstock tags could not be provided. [CA] The identity of the source of shellstock that are sold or served shall be maintained for 90 calendar days from the dates of harvest by using an approved recordkeeping system that keeps the tags or labels in chronological order correlated to the date or dates the shellstock are sold or served.

  • K40: Wiping cloths: properly used, stored

    Multiple dry soiled wiping cloths maintained out on various counters/surfaces throughout the kitchen. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium). Single-use wiping towels shall be removed from the vicinity of food prep/food contact surfaces once they become soiled.

  • K43: Toilet facilities: properly constructed, supplied, cleaned

    Women's restroom door is not capable of self-closing. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.

  • K33: Nonfood contact surfaces clean

    Large amount of accumulated food buildup on the sides of equipment at the cook-line, and on the floor directly underneath the cooking equipment. [CA] Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris to preclude the attraction of pests/vermin.

  • K34: Warewash facilities: installed/maintained; test strips

    Facility lacks test strips for the chlorine sanitizer utilized by their mechanical dishwasher. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing. Obtain chlorine test strips. Mechanical sanitation shall be accomplished via contact with 50 ppm chlorine in a mechanical dishwasher.

  • K14: Food contact surfaces clean, sanitized

    Ice machine observed to have pink/brown mold-like buildup on its interior panel. Buildup not in direct contact with ice. [CA] Ice machine interior panels (i.e. food contact surfaces) shall be cleaned and sanitized regularly, at a rate to preclude buildup and possible contamination of ice.

  • K26: Approved thawing methods used; frozen food

    Raw beef measured 64F-67F in plastic tubs directly in front of the back door. Per staff, they had been taken out of the walk-in to thaw earlier this morning (less than 2 hours prior). Note: [Minor thawing violation leading to a Major holding violation] [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

  • K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing

    1. Employee phone, and multiple open, uncovered employee drinks maintained above/around food prep areas/surfaces. [CA] Employee belongings shall be stored away from open food and food prep areas/surfaces. Employee drinks shall be equipped with tight fitting lids and straws to preclude possible contamination when drinking. 2. Back door maintained open. [CA] Doors shall be kept closed to prevent the intrusion of pests/vermin.

  • K21: Hot and cold water available

    Hand wash sinks in both restrooms are capable of outputting water at a maximum of 75F, as measured at the time of inspection. [CA] Handwashing facilities equipped with a mixing valve that is not readily adjustable at the faucet, shall provide warm water at least 100°F, but not greater than 108°F.

  • K35: Equipment, utensils: Approved, in good repair, adequate capacity

    1. Pipe directly underneath the steamer at the cook-line observed to be leaking. Half-full bowl of water observed underneath leak. [CA] Equipment shall be maintained in good conditions. Repair and maintain. 2. Cardboard is being used to line shelves in front of the cook-line. [CA] Discontinue the use of cardboard to line shelves, as it is not smooth, durable, or easily cleanable, and provides harborage/attraction of vermin. 3. To-go bags are being used to store tea. [CA] Discontinue the use of to-go bags to store food. Foods shall be stored in approved containers with tight fitting lids.

  • K42: Garbage & refuse properly disposed; facilities maintained

    Dumpster lid maintained open. [CA] Dumpster lids (for refuse/garbage) shall be kept covered/closed at all times when not in use to preclude the attraction of pests/vermin.
