This is the first follow up inspection. K06: All hand wash stations were restocked. K07: All PHFs were in appropriate time and temperature relationships. K08: TPHC was being used properly and written procedures were available. K32: Facility has labeled the items at the self service coffee bar. K35: The two door cooler was replaced with a like for like unit and is maintaining PHFs at 41F or below. Facility was in the process of removing the ice machine from the ice bin. K47: Facility has posted the Environmental Health Permit. Continue to correct any and all pending violations.

Notes: - Approved written procedures for TPHC were provided. - Joint inspection with M. Chin. Report written by M. Chin. - A follow up inspection shall be conducted within 3 business days to determine compliance with violations noted. The first follow up inspection is free of charge.     1. Large bulk bin of white substance is not labeled or otherwise identified. 2. Various food items in the consumer self service area are not labeled with their common name. [CA] Food or food related equipment shall be stored in an approved fully enclosed food facility.

    Facility is using TPHC for various items (cheese croissant, hot-dog roll, pesto bread, etc.) but items are not labeled, and TPHC procedures are not available for review. Per the staff, PHF was made an hour ago. [CA] Provide written procedures and time marking for PHFs utilizing time as a public health control (TPHC). When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] PHFs marked for discard after 4 hours of initial cooking/reheating.

    Multiple bottles of sauce observed to be stored on the floor in the kitchen area. [CA] Food shall be stored a minimum of 6 inches off the floor. Raw bacon found stored above bagel dough. PIC elected to use TPHC and will discard PHFs after 4 hours of initially leaving temperature control. [COS] 2. PHFs were discarded. See VC&D report. ____________ Minor violation: Sliced tomatoes measured 43F, and sliced ham measured 45F, in upper prep inserts in the prep unit next to the side door. [CA] PHF shall be held at or below 41F. [COS] PHF in the upper prep inserts were moved to the lower part of the prep unit.

    Repeat Violation: Two non-ANSI approved microwaves observed in use at the front kitchen area. [CA] All utensils and equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). Repeat Violation: Ice machine is non-functional. Facility buys bagged ice and places ice into machine for storage. [CA] Repair, replace, or remove ice machine. Remove the ice maker unit from the bin.

    Multiple soiled wiping cloths observed out on counters. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium). Single use wiping cloths shall be discarded after becoming soiled.
