As of 06/21/2024 routine inspection, plans had still not been submitted, a deadline to submit was given for 07/21/2024. Plans have still not been submitted. [CA] Facility has until 05/06/2025 to submit plans to DEH Plan Check regarding the handwash sink. A permit was issued to this facility on the condition that these plans would be submitted. Failure to comply will result in further enforcement action.

Remove residential freezer. Observed cardboard lining the shelves above the preparation area. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.
