Observed back door of the facility opened. Back doors leads to a patio-like area that is closed off using a metal gate. [CA] Keep back door closed at all times to prevent the entrance and harborage of vermin.

Observed slow drip leak from the ceiling of the kitchen. Water is being caught using small buckets. No contamination of food, food contact surfaces, or food preparation areas observed. PIC stated that a contractor has been scheduled to repair ceiling/roof. [CA] Ceilings of all rooms where nonprepackaged food is handled shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable. * Note: if leaking becomes worse and/or contamination of food, food storage, food contact surfaces, or food preparation areas is observed, facility shall not operate and immediately cease operations until hazard is abated.

Observed rodent droppings in the back area by the ice machines. No other signs of vermin were observed. [CA] Clean and sanitize areas of old droppings.
