FOLLOW-UP INSPECTION

This is the first follow up inspection for the routine inspection where the facility was closed for an active cockroach infestation. A completed and signed checklist for reopening and a pest control report were provided prior to this inspection. Observations: K23: There were no evidence of a cockroach infestation. K45: The following structural deficienies were corrected: - Gaps around pipes under the hand wash and prep sinks. - Untreated wood on the FRP under the prep sink. - Cove base tiles in disrepair near the cook line. - Holes in ceiling tiles in the kitchen. - FRP in disrepair in the janitorial sink room. - Gaps in the underneath areas of cabinets in the server/cashier area. K48: The undercounter dishwasher machine remains to be impounded. It is now disconnected. Plans check review is pending. Facility is okay to reopen.
