This facility shall remain closed until all major violations have been verified corrected by this department and permit has been paid.
Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale.
Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability.

Follow up inspection regarding closure on 08/20/2024 due to vermin (cockroach) activity. Specialist conducted a thorough inspection of the facility and found no vermin activity. Pest control receipt from Pro Active Pest Control provided. Pest control service occurred on 08/24/2024. Per PIC, the facility will be receiving treatments biweekly.
Facility APPROVED to re-open. This is the facility's second follow-up inspection. Facility will be charged the hourly rate of $298.
Continue to follow pest control service recommendations and monitor for vermin activity. Ensure all cracks, crevices, and gaps under any exterior doors to prevent entry and harborage of vermin.

Follow-up inspection regarding closure for improper sewage and wastewater disposal on 08/14/2024. Violation observed corrected.
FACILITY ORDERED CLOSED due to active vermin infestation (cockroaches). Facility shall contact Specialist for a follow up inspection to reopen after all evidence of vermin and vermin activity have been removed and all impacted areas have been properly cleaned and sanitized.
