This limited/routine inspection is conducted for a change of ownership for SR0885177 NEW OWNER: Fola-Fela LLC NEW FACILITY NAME: Folafela The applicant has completed the application for an Environmental Health Permit. FP11 $2,281.00 will be invoiced and mailed to the address on the application. Payment shall be paid within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received within 30 days of the change of owner’s date. Permit issued and valid:07/01/26- 06/30/27. The permit will be deemed invalid if the permit fee is not paid in full within 30 days of the date of this report. This report serves as a temporary permit until the official permit is mailed to the facility. When the invoice for the facility is received, payment must be made within 10 days. Okay to operate as of today. The official Environmental Health permit must post permits from a public view, and a copy of the most recent inspection report must be kept on file for review by anyone upon request. Someone from the facility must take a food manager certificate class within two months. All employees who are engaged in food preparation service, and storage, and who do not have a food manager certificate must have valid food handler cards within 30 days of hire date. Note: The new owner took over the restaurant couple of months ago, and has was found operating without permit ( Fola-Fela Restaurant).. The new owner has brought multiple used and new equipment into the existing kitchen. Part of the dinning area has a shelf built to be used as a grocery store aisle. The new owner is told: 1. To submit a change of ownership application form to our office with copy of the menu within the next 48 hours. The newly added equipment shall not be used until they are approved by our office's plan check unit. The menu shall be restricted to items cooked at the previously approved equipment. Plans for all new equipment shall be submitted to our plan check unit in person on the address written on top left side of the this report or via email to dehplancheck@deh.sccgov.org. Please review the plan check permit application requirement at https://www.sccgov.org/sites/cpd/programs/pcp/Pages/home.aspx. 2. A 25 % penalty shall be automatically added to the annual permit for operating without approved permit. 3. The facility does not have a grocery permit. Therefore, the grocery operation shall cease and desist until the operation is approved by our plan check unit. Please submit layout and specification of the fridges used for the grocery to our plan check unit in person or via email.
