    Observed an electric table top fryer at the sushi area used without an exhaust hood. [corrective action] Fryers must be operated with an exhaust hood. Remove the fryer.

    1. Observed bottle of "Raid" insecticide at the bar. [corrective action] Household insecticides are not approved for use at a public food facility. Provide professional pest control service if needed. Note: Per owner, this facility receives monthly pest service. Owner provided the most recent pest control report by Polaris Pest Elimination dated 04/01/2026. Report noted that pesticides were applied at time of visit and did not note any active pest infestation. 2. Observed gap around water heater vent, where the vent penetrates the roof. Daylight observed coming through gap. [corrective action] Seal the gap around the vent to prevent pests from entering this facility.

    1 of 2 hand sinks at the bar was lacking hand soap. [corrective action] Each hand sink must have hand soap. [correction] Bottle of pump soap was provided.

    Beef tripe is thawed in standing water in 3 compartment sink. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process. [COS] Employee turned on cold running water to thaw beef tripe.

    Portable fryer across from sushi area near walk-in fridge is not used under a hood. A stove burner is placed outside of hood in kitchen. [CA] There shall be a minimum of 6 inches overhang of ventilation hood over cooking equipment. Mechanical exhaust ventilation shall be on while cooking is taking place.

[CA] The identity of the source of shellstock that are sold or served shall be maintained for 90 calendar days from the dates of harvest by using an approved recordkeeping system that keeps the tags or labels in chronological order correlated to the date or dates the shellstock are sold or served.

Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels.

    Several holes on the wall in kitchen and back storage room. Floor in storage room is uneven and observed standing water in some floor tile. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface. Replace broken floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable.
