This is a follow up inspection to verify that all major violations mentioned on 3/2/26 inspection report have been addressed. Major violations have been addressed and found within compliance: - K06: Adequate hand wash facilities supplied, accessible. All hand wash stations within the facility are fully stocked and operational. See measured observations. - K07: Proper hot and cold holding temperatures. Rice has been cooled down and being placed in under counter refrigeration. All PHFs within prep table were removed and unit is still getting serviced by contractor, part is needed for unit. Once unit is operational make sure all foods are held 41*F below at all times. Continue on working on any other violations mentioned on 3/2/26

[SA] Rice was VCD by PIC and thrown into the garbage. 2. Prep table next to the cook line was measured between 49-53*F at the time of inspection. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] Facility will discard all food after 4-hours and have the unit serviced.

    Hood filters within the hood system have heavy debris build up at the time of inspection. [CA] Ventilation hood filters shall be maintained clean and good repair.

    All hand wash stations within the facility have issues at the time of inspection. 1. Hand wash station next to the cook line had equipment inside of it at the time of inspection. [CA] Hand washing facility shall be clean, unobstructed, and accessible at all times. [COS] Equipment was removed from the hand wash station and is now accessible. 2. Hand wash station next cashier station is missing paper towels and soap at the time of inspection. [CA] Hand wash stations must always be stocked with soap, water, and paper towels at all times. [COS] Roll of napkins and hand pump soap was placed next to hand wash.

    Cutting boards around the prep sink have debris build up on them at the time of inspection. [CA] Remove any old cutting boards that have staining or heavy debris build up.

    Ice scooper within the ice machine is in direct contact with ice. [CA] Store scoop handle in such a way that the handle does not come in direct contact with food to prevent contamination.
