1. Found accumulated food debris under refrigeration unit near front service line and under prep table. 2. Found floors in beverage station heavily sticky. Per operator, deep cleaning occurs every 2 weeks. [CA] Routinely clean and deep clean as needed.

Measured 0ppm quat for 2 buckets of sanitizer at prep area. [CA] Ensure to change sanitizer as often as need. [COS] Operator made a new batch of quat sanitizer. Final reading at 200ppm.

Found toilet tissue stored outside of mounted dispenser in employees restroom. [CA] Toilet tissue shall be provided in a permanently installed dispenser at each toilet.

1. Found leaking faucet at hand wash station in front prep area when water knob is of. 2. Found leaking faucet at warewash sink (right side) near cap area, observed water spewing out when water knob is on. 3. Found leaking drain line under hand wash station location at front service line. Observed a bucket used to capture leak. [CA] Repair leak.
