Measured warm water at 113 degrees Fahrenheit at the automatic faucet in the restroom. [Corrective Action] Ensure warm water is dispensed at 100 - 108 degrees Fahrenheit at faucets where hot and cold water are pre-mixed.

Observed water dripping onto floor from the mop head which was leaning against the water heater and wall by the mop sink. [Corrective Action] Hang mop onto the mop hanger and allow mop to drip dry into the mop sink.

Broken floor tiles observed by the proofer. [Corrective Action] Maintain floors in good condition.

Heavy ice build up on the fan guard on the evaporator in the walk-in cooler. [Corrective Action] Ensure equipment is working properly.

Toilet paper observed stored outside of the toilet paper dispenser. Per PIC, the key to open the toilet paper dispenser is missing. [Corrective Action] Store the toilet paper inside of the toilet paper dispenser. Find the key to open the toilet paper dispenser or replace the toilet paper dispenser.

Observed a leak at the bottom of the right compartment of the 3 compartment sink. [Corrective Action] Repair to stop the leak.

Observed a broken floor tile by the proofer. [Corrective Action] Maintain floor in good condition.
