The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,377.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 5/1/26 - 4/30/27. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 4/16/26 *Permit condition: The facility only has one handsink in the warewashing area, and no handsinks in the front food prep areas (staff must walk through 2-3 rooms/corridor area to reach the handsink from the frontmost prep area). If any handwashing violations occur in the future, the facility may be required to install a handsink in the food prep area. Other notes: - The prep sink lacks a drainboard. Operator stated that staff usually cut vegetables on top of a large board placed on top of the 3-compartment sink, then vegetables are washed/rinsed in a large spinner. Specialist informed operator that no food prep/handling is permitted at the 3-compartment sink. Operator proposed that they start cutting vegetables at the prep table adjacent to the prep sink, and then wash vegetables in the prep sink. This method may be revoked in the future (especially if food handling violatoins are observed), at which point the prep sink will need to be upgraded to a type with a drainboard. - Specialist may follow up regarding the upright dishwasher type/possible hood requirement. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date. Prior to the installation of any new equipment, submittal is required to Plan Check.
