ROUTINE INSPECTION

Provided and discussed Checklist for Reopening After Closure Due to Vermin Infestation. Requirements Before Reopening (a minimum 24 hours must elapse before our office can schedule a reinspection): 1. Email the signed and completed Reopening Checklist to the assigned inspector at mindy.nguyen@deh.sccgov.org . 2. Submit a copy of the pest control report from a licensed provider. Clean and sanitize all food and nonfood contact surfaces. Remove all evidence of cockroaches from facility and ensure there is no evidence of vermin found at follow up inspection. ***If ANY cockroaches, dead or alive, are observed during the follow-up inspection(s), the facility will not be re-opened.*** Facility will remain closed at this time and will not reopen until a follow up inspection is conducted. Facility is to cease and desist all food sales and operations, including food preparation. Do not remove, cover-up, or relocate red placard. Failure to comply may result in enforcement actions.     Observed labels prepackaged foods in the front glass door refrigerator was missing information. [CA] Food prepackaged in a food facility shall bear a label that includes: 1. The common name of the food. 2. If made from two or more ingredients, a list of ingredients in order of weight. 3. Quantity of the contents. 4. Name and place of business of the manufacturer, packer, or distributor.

    Inside the walk-in refrigerator: measured internal temperature of pooled eggs and raw shell eggs to be 55F. Note: The walk-in refrigerator door was observed to be open; PIC stated the door had been open for 1 or 2 hours due to replacing parts (the refrigerator was not malfunctioning. This was a parts replacement that was scheduled 3 weeks prior due solely to wanting to maintain it in good condition). [CA] Keep potentially hazardous foods at 41F/45F and below or 135F and above. [COS] Foods were immediately moved to a different refrigerator.

    Observed live cockroaches in the following locations: - 2 live cockroaches on the floor near the handsink in the rear kitchen area - Approximately 6-8 live cockroaches of varying life stages in the mop sink and on the wall behind the mop sink - 1 live cockroach on the wall behind the prep sink [CA] A food facility shall not operate when there is a vermin infestation or has evidence of that is not eliminated or has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s) that cannot be corrected. The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. FOLLOW-UP INSPECTION

This was a follow-up to the 10/08/24 limited inspection to assess compliance regarding violations K13 (adulteration of food) and K23 (vermin): - Observed food was unadulterated and in good condition. - No cockroaches observed during today's inspection. Observed approximately 5 small flies. Continue deep cleaning facility and equipment. PIC stated facility will resume professional pest control services; recommend having pest control services at least once a month.
