    Observed restroom doors lack self-closing devices. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.

    Observed several prepackaged foods for sale on display shelves without proper labeling. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

  • K30: Food storage: food storage containers identified

    Observed food stored directly on the floor in the back storage area and walk-in coolers. [CA] Food shall be stored six inches above the floor to prevent contamination and facilitate ease of cleaning.

    1) Observed several holes in the ceiling above the upright display coolers. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage. 2) Observed storage of unused cooking equipment in the warehouse storage area. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.

    Measured cut wintermelon and cut watermelon at 56F. Per PIC, wintermelon and watermelon were cut and placed into produce cooler less than 1 hour prior to temperature measurement. Measured tofu and sprouts at 51F. Per PIC, tofu and sprouts have been stocked in the produce cooler since overnight. [CA] Potentially Hazardous Foods shall be kept at 41F or below or 135F or above. [COS] Cut wintermelon and watermelon were relocated to the upright coolers to facilitate rapid cooling. The tofu and sprouts were voluntarily condemned and discarded. See VC&D form.

    Measured the ambient temperature of the produce display coolers at 55F. [CA] Refrigeration units shall maintain foods at 41F or below. Discontinue storage of Potentially Hazardous Foods until the unit is repaired.

    Observed broken panels in the wall nearby the drain adjacent to display coolers and in the warehouse storage area. [CA] Close all holes or gaps in the walls. Facility shall remain fully enclosed to prevent the entrance and harborage of vermin.

  • K07: Proper hot and cold holding temperaturesCritical

    At the produce section, measured cut wintermelon at 52F and mung bean sprouts at 58F. Per PIC, products were stocked on the shelves approximately 1.5 hours prior to temperature measurement. [CA] Cold Potentially Hazardous Foods (PHFs) shall be kept at 41F or below [COS] PIC relocated cut wintermelon and mung bean sprouts to a functioning cooler. At the front register section, observed hot holding box for samosas. Measured samosas at 112F. Per PIC, samosa was cooked in the on-site kitchen and moved to the hot holding box 30 minutes prior to temperature measurement. [CA] Hot Potentially Hazardous Foods (PHFs) shall be kept at 135F or above. [COS] PIC turned up the temperature for the hot holding box. Remeasured samosas at 135F.

    Measured the ambient temperature of the produce section coolers at 53F. Observed the display thermometers read at 49F. [CA] Refrigeration units shall maintain Potentially Hazardous Foods (PHFs) at 41F or below. Ensure PHFs are not stored in the produce section coolers until repaired.
