    Damaged floor tiles in front of the mop sink noted at the time of inspection. [CA] Replace broken floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable.

    Sanitizer (chlorine) within the dish washer was measured at 0PPM at the time of inspection. Active ware-washing was taking place. Chlorine was out in container and line was not feeding machine once chlorine bucket was changed. [CA] Maintain chlorine in mechanical ware-wash machine at 50 PPM. [SA] Facility will re-wash any dishes that were placed through the dish washer in the 3-compartment sink to wash, rinse, sanitize.
