Per staff, items had been cooked or taken out of refrigeration 30 minutes prior. Shrimp paste in oil measured 67F in two inserts maintained out of temperature control at the cook-line. Per staff, items had been taken out of refrigeration less than 1 hour prior. Bean sprouts measured 54F and raw shell eggs measured 64F while maintained out of temperature control on a prep table across from the cook-line. Per staff, items had been taken out of refrigeration 30 minutes prior. Two containers of chicken measured 89F while maintained in a turned off steam table on a prep table across from the cook-line. Five containers of chicken measured between 74F and 76F while maintained in a turned off steam on a cart across from the cook-line. Employee observed turning on both steam tables. Per staff, items had been cooked less than 1 hour prior. [CA] Potentially hazardous foods shall be held at 41F or below, or at 135F or above to prevent the growth of bacteria. [COS] Items on the cart and shrimp paste in oil to be served or discarded by the end of lunch service. Going forward, facility has opted to subject items on cart and shrimp paste in oil to time as a public health control (4-hour rule). TPHC form filled out with the owner. Bean sprouts and raw shell eggs placed back into refrigeration. Containers of chicken were reheated to 165F and placed back into hot holding.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Mari Mari NEW OWNER: Srimaju Dining Management LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1377.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 04/01/2026 - 05/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. FOLLOW-UP INSPECTION

On-site for a follow-up inspection after the facility was closed on 10/01/2025 due to failure to provide hot water K21: Hot and cold water not available: Hot water heater has been repaired. Hot water measured at greater than 120F at the three compartment sink and the prep sink. Warm water measured at greater than 100F at all hand sinks throughout the facility. Compliance of the following has been verified: K05: Hands not clean/improperly washed/gloves not used properly - Proper hand washing and glove use observed K06: Inadequate handwash facilities: - All handwash facilities unobstructed and stocked with soap and paper towels. Warm water provided. K07: Improper hot and cold holding temperatures - No PHF's maintained out of temperature control. As per the last routine inspection, all refrigeration units are functioning properly. K13: Food not in good condition/unsafe/adulterated: - No adulteration of food observed. Facility has removed all dried foods from inside the ice machine, cleaned/sanitized the interior, and refilled it with ice. No presence of flies observed. K14: Food contact surfaces unclean and unsanitized: - Proper dishwashing procedures observed. Three compartment sink has been set up as wash, rinse, sanitize. Chlorine sanitizer measured 100 ppm. Facility may now be reopened. Continue to work on the violations cited during the last routine inspection conducted on 10/01/2025.

Facility is hereby closed due to failure to provide hot water. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale A follow-up inspection must be conducted prior to facility reopening. Call or email to schedule. Subsequent follow-up inspections after the first shall be billed at $298/hour, one hour minimum, during normal business hours (7:30 AM to 4:30 PM), or $645 for a minimum of 2 hours, during non-business hours and in accordance with the inspector's availability.
