ROUTINE INSPECTION

Per PIC Lisa, this facility underwent a change of ownership in 2019 (new owner: LPL QUICKBITE INC). However, the current owner name in DEH records is still SOUTH BAY FAST FOODS INC. Within 48 hours, this facilty must provide written documentation that they submitted and paid for a change of ownership with Santa Clara County's DEH, or come to the office and apply for and pay for a change of ownership/facility evaluation application. Failure to comply may result in facility closure for operating without a valid permit.

    The walk-in thermometer's outer thermometer display was not accurate (display read approximately 35F when the inside temperature was 41-45F). [CA] Repair thermometer.

  • K41: Plumbing approved, installed, in good repair; proper backflow devices

    Observed the drain pipe under the soda machine extended into the floor sink. [CA] Air gaps at water supply inlets must be at least twice the diameter of the inlet and no less than 1 inch.

  • K07: Proper hot and cold holding temperatures

    Inside the walk-in refrigerator: measured internal temperature of sliced cheese and juice to be 44F. Ambient temperature measured approximately 45F. Operators stated that there was a lunch rush starting around 12 or 1pm today (approximately 2.5 hours prior); due to a new menu item requiring employees to constantly enter the walk-in freezer (which requires walking through the walk-in refrigerator), the door was opening constantly. [CA] Keep potentially hazardous foods at 41F and below or 135F and above. Note: a metal probe thermometer measured the walk-in refrigerator's ambient temperature to be 41F after it was left inside with the door closed for 20 minutes. Carefully monitor food and ambient temperatures inside the walk-in refrigerator.

    In the rear storage area (where one-time-use cups are stored on shelves): observed accumulation of grease/oil on floor under a shelf. [CA] Keep floor clean.

  • K14: Food contact surfaces clean, sanitized

    Observed pink mold-like substance on the white baffle, left side, inside the ice machine (inspector was able to wipe some off using an alcohol wipe). Ice was not in direct contact with the pink substance. [CA] Keep ice machine clean.

  • K41: Plumbing approved, installed, in good repair; proper backflow devices

    Observed the prep sink faucet leaked water when it was turned on. [CA] Repair leak and keep faucet in good condition.
