OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Danacious Dumpling NEW OWNER: Dan Feng Ran Inc The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $2,281.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 09/01/2026 - 08/31/2027. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 08/04/2026 *Permit condition: N/A *Obtain food safety manager certificate within 60 days. Observed the hand sink across from the cook-line is missing a wall-mounted soap dispenser. Per the old owner, it detached from the wall and has not been replaced yet. *Note: Pump bottle of soap was available. [CA] Install wall-mounted soap dispenser.

Observed a singular dead, dried cockroach in the left-side cabinet at the front service counter. *No other evidence of cockroach activity observed throughout the facility* Pest control service reports were provided for review. [CA] Clean and sanitize area of dead cockroaches or old droppings. Continue to work with pest control.

1. Observed accumulation of grease on the wall above the ventilation hood, and on the wall near the fire suppression system. [CA] Ensure walls, floors, and ceilings are maintained clean to prevent the attraction of vermin. 2. Observed several cracks, gaps, and holes in the walls and ceilings of the facility. Observed one hole sealed with duct tape. [CA] Ensure to seal all structural deficiencies to prevent the harborage of vermin. Discontinue the use of tape to seal; use materials that are durable, easily cleanable, and nonabsorbent.
