Follow-up inspection for dead cockroaches observed during routine inspection conducted 5/5/26. Operator provided most recent pest control reports via email on 5/12/26. The latest pest control report from 4/24/26 notes possible rodent activity inside the facility. At time of follow-up inspection - no evidence of rodent activity observed within the facility. Dead American cockroaches were observed at the following locations - one in the floor sink of the dishwasher, 5 dead on glue trap in the upstairs storage room. No additional signs of cockroaches elsewhere in the facility or by areas of food prep or food storage. Clean and sanitize affected areas and continue regular pest control service.

FOLLOW-UP INSPECTION

Follow-up for facility that was closed due to rodent infestation that resulted in the contamination of food. Facility received pest control service on 3/11/25 at 1 AM. Reviewed service report that notes one rat was caught on a trap. Additional traps has been placed and a daily service will be provided for the rest of the week. - At time of inspection, no vermin activity was observed. - In additional the to affected areas, the kitchen and storage areas has been cleaned and sanitized. - Operator also cleared and removed all excess and unnecessary equipment from the upstairs storage area. - There was no adulteration of food observed. Facility shall continue to seal and eliminate any gaps and crevices that may allow the entering or harboring of vermin. Facility is okay to re-open and resume food operations.
