Follow-up inspection to routine inspection resulting in a closure on 9/03/2025 and a follow-up on 9/05/2025 resulting in the facility remaining closed due to continued vermin activity. Facility has obtained professional pest control services. Reviewed pest control service reports. Facility completed the checklist for reopening after a closure due to a vermin infestation. All contaminated areas have been cleaned and sanitized. No vermin activity observed at the time of inspection. Facility is reopened and may resume operation. This was a charged follow-up inspection. The permit category for this facility is FP17 An invoice for the permit fee in the amount of $2,011.00 plus a 100% late penalty fee of $2,011.00 - total $4,022.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. Follow up inspection conducted for risk factor inspection on 09/03/2025 regarding the following violations: -K07 Proper hot and cold holding temperatures -K23 No rodents, insects, birds, or animals Specialist observations: -K07 Proper hot and cold holding temperatures: CORRECTED; note: drawer coolers were turned off and will not be used until repaired per Manager. -K23 No rodents, insects, birds, or animals: NOT CORRECTED; Rodent droppings observed. Facility is to cease and desist all operations.
