    Facility was found to be using the vac chamber machine to reduce oxygen package numerous PHFs. Items were all either unlabeled or labeled past 48 hours. [CA] The food facility shall obtain a HACCP plan approval prior to packaging PHFs using a reduced-oxygen packaging method. The HACCP plan mus tbe approved by the California Department of Public Health. [COS] Items were discarded. See VC&D report.

    The handwash station at the outside bar lacked a paper towel dispenser. Per staff, it was knocked down this last weekend. Note: A stack of paper towels were observed. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels.

    1. In the middle bar, at the second hand wash station, there was no hot water available. 2. In the women's restroom all three hand wash stations lacked hot water. [CA] Handwashing facilities shall be equipped to provide minimum 100°F water under pressure for a minimum of 15 seconds.

    In the kitchen area, inside the upright freezer, pork was found in Reduced Oxygen Packaging. Per Chef, they pork was intended for "family meal" and they normally use the vacuum sealer for drinks. [CA] The food facility shall obtain a HACCP plan approval prior to packaging PHFs using a reduced-oxygen packaging method. [COS] Chef discarded the pork. See VC&D Report.

    In the kitchen area, at the dishwasher, chlorine measured at 25 PPM. [CA] Maintain chlorine in mechanical warewash machine at 50 PPM.

    At the middle bar, all of the hand washing stations have the permanent soap dispenser in disrepair. A pump bottle of soap is available for use. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times.

    In the walk in cooler, on one of the shelves, raw shelled eggs were found stored above ready to eat foods. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items. [COS] Chef reorganized the items at time of the inspection.

    1. In the liquor storage room next to the middle bar, in a whiskey bottle, a dead fly was found at the bottom. 2. In the front bar, in a bourbon bottle, a dead fly was found at the bottom. [CA] Food is adulterated if it bears or contains any poisonous or deleterious substance that may render it impure or injurious to health and shall be discarded immediately.
