ROUTINE INSPECTION

- Two or more major violations were observed during the routine inspection. - A follow-up inspection will be conducted to verify compliance of all major violations. - Subsequent follow-up inspection after first follow-up shall be billed $282/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. ** Note: - In use preparation surfaces shall be cleaned at least every 4 hours or as required. - Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried.     Observed build up of standing water, grime, and debris on floors throughout the kitchen. Observed dust and grime built up on walls throughout the kitchen. [CA] Walls and/or floors in food preparation area shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

    The only hand wash station in the kitchen completely obstructed with small preparation table with equipment placed in front and other equipment stored on top of the hand wash basin. Hand wash station lacked paper towels and hand soap observed black and appeared to be unused. Owner stated that hands are washed at the dump/rinse sink installed at the front of the kitchen. Hand soap dispenser on the wall lacked hand soap and per owner contains hand sanitizer. Area also lacked paper towel dispenser. Paper towel roll was maintained on the bottom of the preparation table across from the sink. Napkins were provided at hand wash station in the restroom. [CA] Handwashing facility shall be clean, unobstructed, and easily accessible at all times. Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. Hand soap and paper towels shall be readily available and easily accessible at all times. [COS] Obstructions at the hand sink were removed. Towels were provided in dispenser.

    Container of tiramisu cream maintained inside the two-door reach-in refrigerator measured at 70F. Owner stated that the cream was prepared in the morning and was kept out to prepare an order. Numerous containers of milk maintained inside the two-door reach-in refrigerator measured between 44F to 47F. Owner stated that items were recently used to prepare orders. Container of tiramisu cream maintained inside the food preparation refrigerator in the kitchen measured at 71F. Owner stated that the cream was kept out to prepare an order. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Items were relocated into refrigerators to immediately cool.

FOLLOW-UP INSPECTION

This is a follow up inspection to the routine inspection/Change of ownership inspections conducted on 12/02/2025. 1. Floor sink has been installed where previous drain line only existed. 2. Wooden shelving above three compartment sink has been removed and replaced with wire shelving. 3. Coved base in the kitchen, front preparation area and in restroom have been repaired/installed. 4. Textured wall behind front counter has been replaced with smooth tiles. 5. Wooden ceiling slats have been removed and replaced with regular ceiling panels however, ceiling panels are not smooth and easily cleanable. 6. Pork belly sandwiches will be purchased from another location, and sold cold from location, no food preparation will be conducted at facility other than the milk teas and coffee. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: TY Coffee NEW OWNER: Nu Tran The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 01/01/2026 - 12/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 12/02/2025. *Permit condition: Facility is lacking preparation sink therefore, facility is restricted to prepackaged food that doesn't require rinsing, washing, soaking or thawing. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.
