OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: ___EL CHALA INC ______________________ NEW OWNER: _________EL CHALA INC_________________ The applicant has completed the facility evaluation application process for an Environmental Health Permit . The permit category for this facility is FP14 . An invoice for the permit fee in the amount of $1,723.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 03/01/2026 - 02/28/2027 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 02/11/2026 ** Any menu or operation change that results in or requires modification to the kitchen including the addition of any cooking equipment or equipment with plumbing are required to be submitted to this department's Plan Check for review and approval prior to installation and use. Equipment that are added without prior submittal are considered unapproved and will be subject to enforcement action. Observed thermometers missing inside refrigeration units. [CA] A thermometer +/- 2°F shall be provided and correctly located in each hot and cold holding unit containing potentially hazardous foods.

Observed 1 live German cockroach in dining area of facility. No activity observed in kitchen. Observed last pest control service report dated 01/12/2026. Per staff, no cockroach activity has been observed. [CA] Owner eliminated cockroach during inspection. Clean and sanitize area of dead cockroaches. Continue with pest control service to monitor for cockroaches. Any evidence of live cockroach activity will result in facility being ordered closed. Per owner, they will call for pest control service twice a month.

1. 1 door upright refrigerator (TRUE) gasket is torn; top well gaskets observed torn. [CA] Repair gaskets to ensure equipment is working properly. 2. Observed condensate accumulating inside the prep table with top well. [CA] Repair equipment.
