OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: ____LOS MITOTES RESTAURANT_________________________ NEW OWNER: ____CASIMIRA BARRIOS ________________________ The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP 10. An invoice for the permit fee in the amount of $ 1,107 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 9/01/2025 - 08/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on _8/25/25_ ** Any menu or operation change that results in or requires modification to the kitchen including the addition of any cooking equipment or equipment with plumbing are required to be submitted to this department's Plan Check for review and approval prior to installation and use. Equipment that are added without prior submittal are considered unapproved and will be subject to enforcement action. Observed container of oil stored on the floor near the ice machine. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

1. Observed pink/brown organic growth inside the ice machine; no contamination of ice observed. [CA] Clean ice machine by washing with soap and water, rinsing and sanitizing using at least 100 ppm chlorine. 2. Table mounted can opener blade has heavy build up of food residue. [CA] Clean table mounted blade by washing with soap and water, rinsing and sanitizing using at least 100 ppm chlorine. 3. Meat slicer blade has dry food residue. [CA] Meat slicer shall be washed, rinsed and sanitized every 4 hours or less after initial use.

1. Black organic growth observed on the back wall of 3 comp sink. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. 2. Cardboard and aluminum lining observed on shelves. [CA] Discontinue the use of cardboard boxes and aluminum to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

Observed raw shell oysters in the walk-in cooler with no shellstock tag; per PIC, the oysters were removed from the box last night and tag was discarded. [CA] Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty and shall be maintained for 90 calendar days from date of harvest. [COS] PIC voluntarily discarded the oysters in the trash and denatured with chlorine.

1. Observed tile broken/disrepair under the hand sink and on the left side of the cooking equipment. [CA] Repair the floor tiles to prevent entry of vermin. 2. Mops are drying inside the mop sink. [CA] After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Observed accumulation of dust on the walls throughout facility. Observed accumulation of water on floor near the meat slicer; per PIC water is from cleaning. [CA] Walls and/or floors in food preparation area shall be kept clean.

Fruit flies observed near mop room. Observed 1 dead american cockroach near mop sink. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies. Clean and sanitize all impacted areas.
