Damaged floor tiles observed near the blast chiller, fire extinguisher, and damaged cove base by custodial room. [Corrective Action] Maintain floors and walls in good condition.
Dust and grime accumulation on the ceiling in the small walk-in cooler. [Corrective Action] Maintain clean.
ROUTINE INSPECTION
Observed frost/snow accumulation in the large walk-in freezer. Observed lighting a bit dim in the left restroom.
Observed dust accumulation on the ceiling in the small walk-in cooler. [Corrective Action] Thoroughly clean and maintain clean.
Observed mold like substance inside of the ice machine near the ice chute. [Corrective Action] Thoroughly clean and sanitize.
Dust accumulation on the fan guard and evaporator in the small walk-in cooler. [Corrective Action] Thoroughly clean and maintain clean.

Paper towels stored on top of paper towel dispenser at the handwash sinks at the food prep area. [Corrective Action] Store paper towels inside of the paper towel dispenser.
Observed rice cooling in the large walk-in cooler with lids on. [Corrective Action] When cooling PHF's, food may be left uncovered or loosely covered to allow rapid cooling.
Observed dust accumulation on the ceiling and walls in the small walk-in cooler. [Corrective Action] Thoroughly clean and maintain clean.

Loose paper towels observed stored on top of the paper towel dispenser at the handwash sinks in the kitchen/processing area. [Corrective Action] Maintain paper towels inside of the paper towel dispenser.
Dust accumulation on the walls and ceiling in the small walk-in cooler. [Corrective Action] Thoroughly clean and maintain clean.
