    Heavy accumulation of grease and food debris observed on floor and walls around the cook line. Accumulation of debris and grime observed below the ware wash sink and around the undercounter dish machine. [CA] Walls and/or floors in food preparation area shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

    Food and equipment temporarily stored in the hallway of the mall. [CA] Food or food related equipment shall be stored in an approved fully enclosed food facility.

  • K42: Garbage & refuse properly disposed; facilities maintained

    Observed buckets of cooked poultry carcases and opened garbage cans with food maintained outside in the parking lot behind the facility. [CA] Refuse containers shall be covered at all times when not in use.

    The only hand wash station is obstructed with a merchandiser refrigerator in front and a serving tray placed on top of the wash basin. At the time of inspection, facility was still operating while conducting deep cleaning. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Obstruction was removed.

    Two large pots of soup cooling by the rear door of the facility stacked on top of each other. Soup had not reach cooling temperature yet and measured above 135F. [CA] Properly cool PHF's using shallow containers, using no more than 2 inches for solid foods and no more than 3 inches for liquids. Properly store cooling foods in such a way to allow proper air flow.

    Food preparation sink is completely obstructed and covered. [CA] The food prep sink shall be accessible and/or unobstructed at all times. Cardboard used to line floors, shelving, and below cutting boards. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent. Observed the right side door of the food preparation refrigerator detached from the hinge. [CA] Repair door in an approved manner. Equipment shall be in food repair.

    Observed cut cardboard used to cover in-use wooden cutting board. [CA] Properly store utensils and equipment in a clean and sanitary manner; in a way that the working part of the utensil may not become contaminated. Discontinue the use of cardboard.

  • K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing

    Observed inoperable sliding door refrigerator in use to store water bottles and equipment. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.

    The only hand washing station in the kitchen observed partially obstructed with pot lid placed on top of the wash basin. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. Ensure hand wash station is easily accessible at all times.

    One dead cockroach observed on floor in-between the inoperable sliding door refrigerator and merchandiser refrigerator at the front walkway. Owner stated that facility was serviced last week. No other live or dead activity was observed. [CA] Clean and sanitize area of dead cockroaches. * The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.

    Employee's personal beverage observed stored on food preparation counter next to open food. [CA] No employee shall eat or drink in the food prep, food contact storage, or food storage areas, use designated area.

    Observed raw shelled eggs stored above cases of vegetables in the two-door upright refrigerator. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.
