Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM). At time of inspection, facility lacked any type of sanitizer. [CA] Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. ** Facility is to immediately cease and desist all operations until an approved sanitizer is provided. [COS] Owner provided chlorine bleach. Three-compartment sink was set up for manual ware washing. Instructed employee to rewash and sanitize all previously washed utensils.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Banh Cuon Hong Hanh NEW OWNER: Nhan Tran The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $1,580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days.
